Medical Records Issuance Services

Regulation for reading and issuing medical records
In accordance to Article 21 of the Medical Service Act, no medical person or any person working for a medical institution may release the details of a patient's records or make them accessible, such as providing a copy or making it available for reading.

Anyone who violates Article 88(Penalty Provisions) shall be punished by imprisonment with labor for not more than three years or by a fine not exceeding 30 million won

Valid ID : Resident Registration Card, Alien Registration Card, Driver's License, Passport, other ID issued by a public organization※ Certificate of Imprisonment(Only valid for the date of issue) for prisoners, and Certificate of Military Service are equivalent to patient ID. Therefore, consent form and power of attorney signed by the patient must be provided.
Proof of family relationship : Certificate of Family, Certificate of Resident Registration, other documents proving family relationship.
Family(Spouse, Parents, Children, Parents of Spouse)
Only original copies of consent form and power of attorney signed by the patient shall be accepted. It will only be valid for 7 days from the date of signature.

Consent to Issuance of Copies of Medical Records and Power of Attorney
- In accordance to the Medical Service Act, a representative(patient, spouse, parents, children or a third party designated by the patient) applying for copy of medical records on behalf of the patient must provide a consent form and a power of attorney signed by the patient.
- In case of patient's death, unconsciousness(only relatives) and patient being a minor(legal guardian) the patient's legal guardian or a third party representative may request access to medical records on behalf of the patient.
- However, issuance of medical records may be withheld if it is deemed necessary for the purpose of the patient's treatment. (eg. Informing the patient with the details of his/her medical condition will have a critical effect on the patient's psychological state.)
