Certification Issuance Guide

Certificates can only be issued to the patient.(Show identification)
If are a family member issuing the certificate, you must provide your ID, patient's ID(or a copy), proof of family relationship, and a consent form signed by the patient.
If you are not a family member, you will need to provide your ID, consent form and power of attorney signed by the patient and patient's ID(or a copy).
Fees for issuing the certificates are not covered by insurance.

General medical Certificate

A general medical certificate without specific format or requirement.

Issued only to the patient

Confirmation of Hospitalization & Discharge

Certificates that do not require a diagnosis can be issues at the Medical Records Issuance on the 1st floor.
For submission to insurance companies, please check with your insurance company whether a diagnosis must be included. In this case, you will need to request a medical certificate.

Health Examination Certificate for Military or Employment

Please contact the clinical department prior to your visit or military purpose, you will need to bring 2 photos and an ID for each department.
For employment, please bring 1 photo and an ID.

Death Certificate

Death certificate can only be issued when the patient dies during ER or inpatient treatment.
Death certificate can be issued if the patient dies within 48 hours after returning home under declaration of death by the attending doctor.
Please make sure you provide the correct address, RRN, and contact information.

Birth Certificate

Birth certificates are issued on the day of mother's discharge. Document can be received from the nurse's office.

Certificate of Injury

Certificate of injury can be issued when injury due to an accident needs to be proved.

Medical receipt for tax deduction

Medical receipts for annual tax deduction is issued by the outpatient services.
Certificates can be issued through postal service or fax as well. Receipts are issued free of charge.

Certificate of Payment and Detailed list of medical bill

Prescription

2 copies of prescriptions can be issued at the payment desk which you can submit to the pharmacy to purchase your medication.
Prescriptions are valid for 7 days from the date of issuance. You must visit the pharmacy within 7 days.
If the date expires, you will need to consult your doctor to reissue the prescription.

ER Patient Medical Certificate

Please inform the nurse at the ER after making your payment to issue a medical certificate from the ER.

Copy of medical records and images(CD)

Copies of your medical record and images(CD) can be issued at the medical record issuance desk on 1F.
