• Please fill out the Patient Inquiry form and send it to us via e-mail along with your medical records, examination results, diagnostic images(X-ray, CT, MRI, etc.) in DICOM format and pathological reports.
• It is very important to provide as much information about your past and current health condition regarding your illness in order to conduct the initial review and provide you with as much detail regarding your treatment plan and estimated cost.
• We do encourage patients to provide all related documents in English.
  However, we do receive documents in Russian. In this case, it may take more time to respond to your inquiry due to time required for translation.
  Also, please make sure that all documents are in typed format.
